Occupancy Flashes on Sensing but stays off until manual select

I have single pole MS-OPS2H occupancy sensor installed. No neutral. I have grounded both green and bare to the metal box.

When unit senses motion, half the lights flash and turn off. When I press the button manually nothing happens the first time (presumably it turns off the sensor). The lights always come on upon second press. Then the sensor turns off by itself after timing out in 5 minutes.

Is this a faulty switch? Should I just exchange it and try again?

I have a similar Lutron (occupancy plus dimmer) wired up in a different section of the basement the same way and it works fine with dimming. The basement was finished about 5 years ago with new wiring and breakers. The village approved the wiring and they are very strict on code.

I would not say immediately it is a bad sensor. We would recommend calling us while on site with a multi meter, at 1-844-588-7661 option 1. By troubleshooting over the phone we can see if it is a grounding issue, wiring issue, or sensor issue.
