Unable to pair Lutron Caseta dimmer with the lutron base

I added a lutron Dimmer to the lutron base through my phones app. I was seeing some oddities when turning the switch on and off so I removed it from the lutron base/app. No matter what I do I cannot join this switch back the the lutron base/app. I restored the switch to factory settings but still am unable to add it back. Pairing the switch with a pico works great. I am our of ideas. When I goto add the switch in the app all my existing caseta switches blink during the add process. What type if fixture/bulb are you controlling with the new dimmer?

4 4in Led cans. That works fine. Just cannot get the switch to pair with the lutron hub to control from my phone

Try a factory reset of the dimmer:
Triple tap quickly, and then hold the on button until the lights flash.
Release and triple tap again within 1/2 second.

Is that the same way you did the first factory reset?
