Caseta Motion sensor: intermittent switch shows as off but stays on

After remodeling a bathroom, I'm restoring Caseta motion sensor operation of three dimmer switches. Operation was flawless before remodel, but now, one switch sometimes remains "on" after motion sensor instruction to "off," even though the app shows the switch is "off". An app press of the switch "off" button does turn the lights off.

This room is farthest in the house from the hub, but had been (and should still be) served by a lamp-switch range extender.

We've had some odd GFI trip and reset issues that would not seem to be related.

Any suggestions for dealing with an intermittent failure-to-turn-lights-off, even though the app shows "off" status?

Hello snerdlybosco and welcome to the forums!

Apologies for this intermittent sensor performance. Just some clarifying questions:
    - Does the dimmer respond consistently from the lightbulb button on the top of the sensor?
    - Can you confirm the PID (plug-in dimmer) is being identified as an extender in the Lutron app? Please tap Settings > Advanced > Range Extender.
    - How far away (in feet) is the PID from the Hub and dimmer that is failing to respond?
    - Is this dimmer on any Scenes or Schedules? If so, is it responding properly?
