PD-5ANS has power but does not turn on light

Installed the PD-5ANS to a single wall location. The LED dot on the switch is green. When I touch the ON button, the LED dot becomes a brighter green, but the light does not turn on. WHen I touch the OFF button, the LED dot goes back to the original less brighter green.

I tried pulling out the FASS (Front Accessible Service Switch) out on the switch to the OFF position and turn on the switch. The LED dot goes to the brighter green. Lights stay off as expected. Then I push in the FASS back in to turn on power to the light. THe lights flash on and then off immediately. Light stay off.

Please help with suggestions. Thanks.

You might have the wires incorrect. Try switching the wire in to the switch with the wire out of the switch.
